KMAK INFO
Welcome to KMAK 2011
This form will help you manuever through the process of registration:
- On or before January 1, 2011 print off and send in your $20.00 per person deposit with pre-registration form. This is a NON-REFUNDABLE deposit. This will hold your spot for KMAK.
- Each person will need to fill out the individual registration form. Make one copy. The camp gets the original, and the church has a copy.
- Make copies of Sponsor Application for each adult attending, and bring to the kamp meeting two weeks prior to your week of kamp.
- Make copies of Medical Authorization Form for each child bringing medication. We will collect all medication and forms on Monday morning during registration at kamp. This form should be in the gallon size ziplock bag with the original container for the medication listing the child’s name and the church they are attending with.
- For your parent meeting make copies of “what to bring list” for each parent/child.
- Your week of KMAK will have a meeting prior to kamp at FBC Tulsa’s Detroit Tower. The address is 434 South Detroit, Tulsa Oklahoma. At this meeting bring the t-shirt form completed. We will give you name tags, students handbooks, counselor handbooks, and t-shirts. (Counselor handbook can be downloaded from www.tulakogee.com if you need a copy before the meeting.) If you have individual registration form filled out we will take them at this time or at kamp on Monday.
