KMAK Church Leader Check List
My Check List for KMAK
□ Pick the week for KMAK you would like to attend.
□ Send pre-registration form and your non-refundable deposit of $20.00 per person to:
□ Make copies of the Individual Registration Form for each person and sponsor attending KMAK (make 1 photo copy of completed form, the Camp keeps the original.)
□ Make copies of Medication Authorization Form to be filled out by parents who have Kampers who need Meds administered. Have all medicines in original container and placed in a one-gallon size zip-lock bag with childs name and church name printed on front.
□ Make sure each sponser has had a background check. Each church is responsible for having the bacground checks done for each sponser. Tulakogee just has to have confirmation from the church that the proper checks have been done.
□ Note the date of mandatory Sponsor Training meeting at 6:30 p.m. at First Baptist Church Tulsa’s Student Center. FBC Tulsa is located at 403 S Cincinnati Ave Tulsa, OK 74103-5048.

□ Find willing sponsors to help with activity classes.
□ Have Group Registration & T Shirt Form filled out at time of Sponsor Training meeting.
□ Have final payment ready and bring to Sponsor Training meeting with checks made out to Tulakogee.
Any adjustments will be made the first day of your week of KMAK.
