Welcome to KAMK 2009

This form will help you maneuver through the process of registration.

1) On or before January 1, 2009 print off and send in your $20.00 per person deposit with Pre-Registration Form. This is an non refundable deposit. This will hold your spot for KMAK.

2) Each person will need to fill out the individual registration form. Make one copy. The camp gets original and the church has a copy.

3) Make copies of Sponsor Application for each adult attending, and bring to camp meeting two weeks before your week of camp.

4) Make copies of Medical Authorization Form for each child bring medication. We will collect on Monday morning at registration. This form should be in the gallon size zip lock bag with the original container. Child’s name and name of church name.

5) Your week of KMAK will have a meeting Tuesday night two weeks before camp at the first Baptist Church Tulsa, 403 S Cincinnati Ave, Ok 74103-5048. At this meeting bring the T-Shirt form filled out. We will give you name tags, students handbooks, Counselor handbook and t-shirts. (Counselor handbook can be down loaded from web site if you need a copy before the meeting.) If you have individual registration form filled out we will take them at this time or at camp on Monday.

6) For your parent meeting make copies of “What to bring list” for each child.
