My Check List for KMAK 2008

□    Pick the week for Kamp

□    Send pre-registration form and your non-refundable Deposit of $20.00 per person to:

  □   Make copies of the Individual Registration Form for each person and sponsor attending Kamp (make 1 photo copy of completed form,  the Kamp keeps the original.)

□    Make copies of Medication Authorization Form to be filled out by parents who have Kampers who need Med’s administered.   Have all medicines in original container and placed in a one gallon size zip lock bag.

□  Make copies of Sponsor Application form and have all sponsors fill out and return to you to bring to Sponsor Training meeting.

□    Note the date of mandatory sponsor training meeting at 7:00 p.m. at First Baptist Church Tulsa located at 403 S Cincinnati Ave Tulsa, Ok 74103-5048. 

□    Find willing sponsors to help with activity classes.

□    Have Group Registration & T Shirt Form filled out at time of sponsor training meeting.

□    Have final payment ready and bring to Sponsor training meeting with checks made out to Tulakogee.
